Job Summary
Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician. Evaluate patient needs to improve mobility, strength, balance, and overall function, then document all visits, assessments, and progress notes accurately. Collaborate with an interdisciplinary team including nurses, occupational therapists, and social workers to provide coordinated care. Manage geriatric, orthopedic, or neurologic rehab for post-acute and chronic conditions while supporting patients in maximizing independence at home. Requires a Michigan PT license, Master's degree, and 1+ year of experience in hospital or home health settings. Full-time role with a $7,500 sign-on bonus, fleet car program, and comprehensive benefits including medical, dental, vision, and 401k match.
Required Qualifications
- PT Licensure - Michigan
- Master's degree in Physical Therapy from an accredited program
- Current PT license in the state of employment (or eligibility to obtain)
- Valid driver's license, auto insurance, and reliable transportation
- 1+ year of experience in a hospital, home health, or outpatient rehab setting
- In-home therapy delivery
- Geriatric rehab, orthopedic or neurologic rehab
- Post-acute care or chronic condition management
- Working as part of a mobile or interdisciplinary care team
