Job Summary
Medical Matron / Nurse Practitioner role performing primary care for frail elder participants within the Senior Community Care of Michigan PACE program. Under Medical Director supervision, conducts physical assessments, semi-annual reassessments, develops and implements plans of care, evaluates complaints, and coordinates with Principal Physicians and Interdisciplinary Team to integrate care plans. Requires RN licensure, NP certification, geriatric-focused experience (desirable), and travel to participant homes with occasional on-call after hours; full-time schedule Monday–Friday 8:00a–4:30p with local travel. Emphasizes collaboration, discharge planning, chronic disease management, and comprehensive geriatric care in a VOANS setting, with emphasis on high-quality, person-centered elder care.
Required Qualifications
- Licensed as Registered Nurse (R.N.)
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- Shall have either training or related experience in the job assigned
- Certified as a Geriatric Nurse Practitioner (desirable)
- 2 years' experience as a nurse practitioner in a geriatric setting desirable
