Job Summary

Coordinate medical follow-up for youth by scheduling appointments, documenting provider recommendations, monitoring conditions, and managing prescription orders. Assist the Nurse Coordinator with scanning documentation into the EMR, inventorying supplies, and administering medication per established procedures. Provide direct supervision and care to individuals, including mentoring staff, ensuring daily living skills, and maintaining a safe, sanitary environment. Serve as shift charge, document targeted behaviors, and provide transportation to appointments or home time as directed. Ensure compliance with OPWDD and HCBS regulations while applying crisis intervention principles and physical restraints when necessary. Work a variable schedule including evenings and weekends.

Required Qualifications

  • High School Diploma or GED
  • Minimum 18 months of related work experience
  • Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record
  • Minimum insurance coverage that meets agency standards
  • Successful completion of Approved Medication Administrative Personnel (AMAP) training within 3 months
  • CPR certification
  • Ability to recognize and address escalating and dysregulated youth behavior
  • Utilization of training provided in behavior management and Strategies for Crisis Intervention and Prevention (SCIP)
  • Use of physical interventions (holds)
  • Ability to meet the highest standard for ethical and professional conduct
  • Manual dexterity including the frequent ability to grasp in both hands and continuous use of fine manipulation skills
  • Ability to work a variable scheduling including evenings and weekends
  • Weekly lifting up to 50 lbs
  • 10-15 lbs lifting on a daily basis
  • Weekly carrying up to 55 lbs approximately 10-100 ft
  • 10-15 lbs carrying on a daily basis
  • Close-toed shoes
  • Protective shields
  • Universal precaution PPE
  • Gloves for toileting and food preparation
  • Ability to physically restrain clients weighing between 50 and 200 lbs
  • Ability to guide clients safely to the floor
  • Ability to change positions as needed
  • Ability to work up to 8 hours of driving
  • Ability to work up to 8 hours of continuous balancing
  • Ability to work up to 2.5 hours of balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting
  • Ability to work up to 2.5 hours of walking
  • Ability to work up to 1 hour of standing
  • Ability to work up to 4 hours of sitting
  • Ability to work in environments with occasional exposure to changes in temperature, dust, fumes, gases or chemicals
