Medical Foster Care Nurse Case Manager-Brooklyn/Staten Island

Nurse Case Manager for Medical Foster Care responsible for coordinating care for a caseload of medically fragile or special needs children residing in foster boarding homes. Duties include conducting mandated monthly home visits, providing medical training for foster and biological parents, accompanying foster parents to medical appointments, and advocating for children’s needs. Responsibilities also cover creating and maintaining individualized care plans, coordinating appointments, managing supplies and medications, triaging calls, administering immunizations, maintaining HIPAA-compliant records, and collaborating with NP/MD, social workers, and other healthcare providers. Requires strong nursing competencies, ability to train families on medical conditions and safety, proficiency in electronic documentation, and excellent communication skills. Bilingual (Spanish/English) strongly preferred. Minimum qualifications include NYS RN licensure, an associate degree, pediatric and case-management experience, and the ability to work in-person with flexibility for schedule.
