Job Summary
Coordinate care for a caseload of fifteen or more medically fragile children in foster boarding homes through mandated monthly home visits, medical training for parents, and accompanying foster parents to specialty appointments. Develop and maintain individualized care plans, administer immunizations, and triage daily phone calls while serving as an emergency on-call resource. Manage medical records, communicate with interdisciplinary teams and community providers, and complete required reports and school forms. Accept 24-hour beeper coverage for four weeks annually.
Required Qualifications
- NYS Licensed Registered Nurse
- Associate's degree
- Minimum of two years of experience in a hospital type setting (e.g., long-term care facilities)
- Pediatric experience
- Case management experience with children who are medically fragile
- Excellent verbal and writing skills
- Comfort in electronic documentation and communication on a variety of platforms, including email, text messaging, and chat-based platforms
- Goal oriented with strong time management skills
- Flexibility and ability to work under pressure
- Accepts 24-hour beeper coverage for at least four weeks annually
- In-person collaboration
- Comply with all privacy laws including HIPAA
