Medical Doctor - Primary Care

Job Summary
Primary Care physician (PCP) role within a PACE Organization, providing comprehensive medical care and leading/participating in an interdisciplinary team to assess and manage participants in a nursing home setting. Responsible for initial and ongoing assessments, care-plan development and implementation, consultations with medical specialists, coordination of inpatient and outpatient care, and regular communication with nursing home staff. Maintains documentation, uses diagnostic coding, reviews outcomes/data, supports policy development and quality assurance, and contributes to program expansion with resource planning. Requires active MD/DO license, CDS and DEA, BLS, and Board Certification (Internal Medicine or Family Practice; geriatrics preferred). Minimum 1 year experience with frail or elderly populations; valid driver’s license and transportation. Periodic regional travel expected; strong interpersonal and organizational skills essential; proficient with Microsoft Office and healthcare software.
Required Qualifications
- Doctorate Degree (MD or DO) and active physician license in state of PO
- CDS Certification and DEA Registration
- Current BLS required
- Board Certification Internal Medicine or Family Practice; Geriatrics preferred
- Minimum of one (1) year working with frail or elderly population
- Valid driver's license, proof of insurance, and means of transportation
- Ability to work with minimal supervision and exercise independent judgment
- Strong communication and collaboration skills; ability to work in a virtual, collaborative shared leadership environment
- Proficiency with Microsoft Office (Word, Excel, PowerPoint) and related software
