Job Summary
Serve as the attending physician for patients in each assigned skilled nursing facility, collaborating with NPs/PAs to manage daily patient care and oversee clinical supervision. Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions, while participating in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives. Cover each facility 1–2 days per week with no fixed start or end times, freedom from on-call and overnight requirements, and access to an EHR with voice dictation and AI Scribe. This role supports Theoria's mission of building trust with vulnerable populations through flexible scheduling and intuitive technology.
Required Qualifications
- Physicians (MD/DO)
- Active U.S. medical license
- Board-certified or board-eligible in an appropriate specialty
- Board-eligible and actively pursuing licensing (for new graduates)
- Specialty in Internal Medicine
- Specialty in Family Medicine
- Specialty in Emergency Medicine
- Specialty in General Practice
- Specialty in Physical Medicine and Rehabilitation
- Specialty in Geriatric Medicine
- Ability to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation
- Ability to pass criminal background checks
- Ability to pass pre-employment drug testing
