Job Summary
Conduct community-based physician rounding in subacute skilled nursing facilities, leading clinical oversight and quality for PACE provider services. Collaborate with an experienced interdisciplinary team to champion person-centered care for frail elderly participants while serving as a physician leader and mentor to NPs and other providers. See 10–20 patients at a time once weekly with only two call days per month. This role offers flexible scheduling, a $5,000 annual CME allowance, and a competitive salary commensurate with experience within a mission-driven organization dedicated to helping vulnerable seniors live independently in Colorado.
Required Qualifications
- MD or DO
- licensed to practice in Colorado
- ABMS Board Certified — Internal Medicine or Family Medicine
- DEA registration
- ability to maintain hospital privileges
- Experience in primary care with frail or elderly populations
