Job Summary
Coordinate and schedule all medical and psychiatric services for adults with developmental disabilities, including routine, follow-up, and emergency care. Confirm appointment locations, arrange transportation, and ensure all necessary documentation and scripts are ready prior to leaving for each visit. Review the daily medical appointment calendar, attend appointments with full attention, and obtain completed consultation forms and physician notes. Complete proper treatment documentation and communicate updates to all appropriate team members. Maintain medication inventory and manage paper or electronic filing systems to ensure all required documentation meets quality and compliance thresholds. Travel between FREE locations and doctors' offices as needed.
Required Qualifications
- High school diploma or equivalent
- Minimum of one (1) year of experience providing services to individuals with disabilities
- Valid AMAP certification
- Proficient computer skills
- Proficient writing skills with grasp of basic grammar and spelling
- Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs

- Excellent communication skills
- At least 18 years of age or older
- Valid driver's license
- Ability to satisfy criteria to be an agency driver
- Ability to sit, walk and stand for prolonged period of time (at least 8 hours)
- Ability to effectively communicate both verbally and in writing
- Ability to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets
- Ability to reach with hands and arms
- Ability to bend, stoop, kneel, crouch or stand on a stool as needed
- Ability to lift 50 pounds at a time
