Medical Coordinator (LVN) - Day Shift

Perform initial intake and conduct head-to-toe assessments for patients arriving at recuperative care centers, establishing care plans and setting weekly goals for medical or social reintegration. Conduct morning, afternoon, and evening checks to monitor client wellbeing, arrange transportation to appointments, and coordinate home health visits as needed. Enter patient information into EHR systems, update medication lists and progress notes, prepare billing reports, and maintain proper inventory of medical supplies. Report emergency situations, document incidents, and ensure compliance with HIPAA, OSHA, and DHS standards while promoting health literacy through patient education. Work Thursday through Monday from 7:00am to 3:30pm with available weekend and night shifts at facilities across Orange, Los Angeles, and Inland Empire counties.
