Job Summary

Oversee medication dispensing and documentation in accordance with AMAP procedures, including maintaining appropriate stock, preparing monthly MARs, and ensuring timely refills. Conduct weekly nursing coordination with the residential nurse, schedule medical appointments, and accompany individuals to appointments to collect paperwork and submit documents for review. Provide hands-on assistance with daily living tasks, room cleaning, food preparation, and personal care while implementing residential habilitation services and behavioral support plans. Assist with transportation using agency vehicles and supervise individuals within the program and community. Complete assigned reports, logs, and documentation per ACLD Corporate Compliance Policy, and implement SCIP maneuvers when necessary. Work Sun-Thurs 8a-4p; $40/hr. Requires valid New York State driver's license, AMAP and SCIP certifications, and one year experience with developmentally disabled populations preferred.

Required Qualifications

  • High School Diploma or equivalent
  • Clean, valid New York State driver's license
  • Ability to communicate and understand in English; both verbally and in writing
  • The ability to obtain and maintain AMAP (Approved Medication Administration Personnel) Certification
  • The ability to obtain and maintain SCIP (Strategies for Crisis Intervention and Prevention) Core Certification
  • CPR (Cardio-Pulmonary Resuscitation) Certification
  • First Aid Certification
  • AED (Automated External Defibrillator) Certification
  • The ability to assist with lifting and transferring a person
  • The ability to lift up to 15 lbs
  • The ability to reach, bend, twist, kneel, and squat as you assist people with physical activities
  • The ability to perform SCIP maneuvers which may require bending, twisting, kneeling, crouching, lying on the ground, and supporting another person's body weight
  • Substantial reasoning skills required including the ability to deal with a variety of abstract and concrete issues simultaneously
  • The ability to identify problems, interpret data and draw conclusions
  • The ability to apply principles of logic or theoretical thinking to a wide range of intellectual and practical problems
  • Excellent verbal and written communication skills including the ability to coach and counsel, interpret and advise, and the ability to professionally assess and resolve problems
  • Ability to provide clear and legible documentation on records
  • Ability to perform basic computer skills
  • Ability to assist people supported in performing daily living tasks, including, but not limited to, room cleaning and household chores
  • Ability to assist the people supported with transportation utilizing agency vehicles
  • Ability to implement SCIP (Strategies for Crisis Intervention and Prevention) maneuvers when necessary
  • Ability to attend required staff meetings, supervision sessions and training programs as assigned
  • Ability to maintain collaborative relationships with other ACLD programs and departments to maximize service coordination and integration

  • Ability to adhere to Agency systems designed to promote organizational efficiencies as well as compliance with ACLD policies, procedures and regulations governing programs
  • Ability to supervise people supported within the program and community
  • Ability to complete and review assigned reports, logs, and documentation in accordance with ACLD Corporate Compliance Policy
  • Ability to assist the people supported with the use of their personal allowance, including oversight in shopping, budgeting and managing their income
  • Ability to interact positively with families, Care Managers and other outside providers while assisting the people you support in developing and maintaining relationships
  • Ability to represent ACLD in a positive manner
  • Ability to oversee the dispensing and documentation of medication in accordance with AMAP procedures and follow all AMAP guidelines with regards to conducting medical appointments as assigned
  • Ability to maintain appropriate stock of medications for each person supported, including OTC, etc
  • Ability to meet with the residential nurse to conduct weekly nursing coordination
  • Ability to schedule all medical appointment follow ups as directed
  • Ability to attend medical appointments with people supported, schedule follow up, collect all paperwork, submit to nurse for review
  • Ability to communicate with families regarding medical appointments as needed
  • Ability to prepare the MARs on a monthly basis
  • Ability to review all MARs and corresponding documents for accuracy
  • Ability to ensure refills of medications are ordered in a timely manner to prevent out of stock issues
  • Ability to ensure new prescriptions are obtained in a timely manner as per procedure
  • Ability to ensure all documents are prepared and accompany person supported on medical appointments (medical treatment summaries, diet orders, prescriptions requests, lab orders, Nurse's notes, etc.)
  • Ability to complete medication administration error reports as needed
  • Ability to implement and document the delivery of all Residential Habilitation services which may include but is not limited to: person supported skill development training in a variety of areas including: medical concerns, vocational concerns and recreational services
  • Ability to implement of all person supported behavioral support plans/strategies and documentation of person supported behaviors
