Job Summary
Grant Funded Medical Case Manager role leading an infectious disease team. Responsible for provisioning and evaluating services delivered by assigned Medical Case Managers across the infectious disease division, managing a defined caseload, and providing leadership and mentoring to staff. Serves as Team Leader coordinating day-to-day operations, assists with hiring/training of new medical case managers and social work interns, and oversees MCM operations in the absence of the director. Requires a Master's degree in Social Work or related field and at least 3 years of ambulatory/acute care experience, with strong organizational, leadership, and regulatory knowledge to support quality patient care at Prisma Health in Columbia, SC.
Required Qualifications
- Master's degree in Social Work, Counseling or health/human services related field
- Three (3) years of experience in ambulatory or acute care setting
- Knowledge and skills to provide clinical and administrative leadership to assigned team
- Knowledge of Healthcare Reform, government and third-party reimbursement systems and contracts
- Organizational, time management, problem solving, and critical thinking skills
- Understanding of pathways, procedures, and governmental regulations
- Knowledge base representative of the skills and training of a MSW Case Manager preferred
