Medical Case Manager (MVP)

Job Summary
Provide medical case management for a designated caseload of 25–34 patients; conduct thorough needs assessments for medically vulnerable patients; develop and update individualized, patient-centered care plans; coordinate and facilitate delivery of prescribed medical and supportive services; arrange transportation for patients to access services both within the clinic and with community partners; collaborate with interdisciplinary teams and external service providers to ensure continuity of care; maintain accurate patient documentation and meet medical/data reporting requirements; stay current on clinic policies, procedures, systems, and electronic health records; participate in weekly check-ins with the MVP Clinic Manager; support a healthy work environment and uphold FSC's values, fostering respect, trust, and collaboration.
Required Qualifications
- High School Diploma or equivalent (Bachelor's degree in a healthcare-related field preferred)
- Experience in medical case management or related patient support functions (preferred: 2+ years in ambulatory care)
- Strong attention to detail and commitment to accuracy in all work
- Ability to work collaboratively and demonstrate a caring, compassionate approach to patient care
- Bilingual in English and Spanish (preferred)
- Experience working with medically vulnerable or unhoused populations (preferred)
