Job Summary
Medical Case Manager II coordinates and delivers comprehensive case management services to clients affected by HIV, Hepatitis C, STI, and other infectious diseases. Responsibilities include eligibility screening, care planning, resource linkage, and collaboration with healthcare teams to promote optimal health outcomes. Perform comprehensive assessments across health, mental health, social support, addiction, financial resources, benefits, legal, language/culture, and employment; verify insurance coverage and coordinate pre-authorizations; develop and update individualized care plans; act as liaison between patients and care teams; ensure access to primary medical care; document activities per grant standards; conduct biannual reassessments for continued enrollment. Travel to Mohave County satellite location (at least 3 days/week) with support visits to the Las Vegas clinic on flex days.
Required Qualifications
- Bachelor’s degree in social work, Nursing, Public Health, or related field OR 4/7 years’ experience
- Three years of HIV prevention/intervention experience
- MCM Certification for TGA, or able to obtain within 90 days
- Must be able to operate a motor vehicle and have valid insurance and driver’s license
- Must be able to pass a Level I and Level II Background check as required
