Job Summary
Complete intakes and ongoing assessments with clients to identify needs, develop goals, and access resources for individuals living with HIV/AIDS. Monitor and document client contacts, progress, and significant occurrences per grant guidelines. Facilitate coordination with internal medical, dental, and mental health services, as well as external agencies for referrals and advocacy. Ensure clients have transportation to necessary appointments and conduct outreach at community agencies, shelters, and neighborhoods. Communicate with community partners using professional discretion while obtaining appropriate information releases. Perform other duties as assigned, including assisting clients with entitlement program applications.
Required Qualifications
- Bachelor's degree in psychology, counseling, mental health, human services or other related field or equivalent combination of education experience
- Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the State of Iowa
- Active Chauffer's license from Iowa Department of Transportation
Desired Qualifications
- Six months of work or volunteer experience in human services related activities
- Work experience with vulnerable populations, including people experiencing homelessness
- Demonstrated competency using HMIS or CareWare as applicable to department
- Working knowledge of community resources related to homelessness or HIV/AIDS in Des Moines
- Knowledge of program terminology
- Ability to effectively use HMIS or CareWare system to document all client contact
- Demonstrate language proficiency in Spanish
