Medical Case Manager I

Coordinate and deliver comprehensive case management services to clients, screening eligibility, developing care plans, linking to resources, coordinating insurance and pre-authorizations, and liaising with care teams to promote optimal health outcomes. Perform comprehensive assessments across health, mental health, social support, addiction, finances, benefits, language/culture, and employment. Attend client medical appointments as needed and accurately document all encounters within software, ensuring timely monthly billing documentation in accordance with grant standards. This role supports patients affected by HIV, Hepatitis C, STI's, and other infectious diseases within a nonprofit healthcare setting.
