Medical Case Manager-HIV, Infectious Disease, FT, Days

Conduct outpatient HIV medical case management and patient advocacy through education, counseling, and benefits assessment. Ensure patient access to essential medications and support services by coordinating services within the Ryan White Program. Perform treatment adherence counseling, manage intake and assessment of service needs, and develop individualized action plans. Monitor client efficacy and handle benefit enrollments for Medicaid, Medicare, ACA, and ADAD. Provide coverage in the duty room to assist with Patient Assistance program applications. Serve as a member of the interdisciplinary health team. Work full-time days at Prisma Health's Infectious Disease facility in Richland.
