Job Summary
Coordinate services for HIV-positive clients in Project SHINE; provide case management and care coordination as part of clinical care teams, linking clients to medical and psychosocial services, coordinating treatments, and developing personalized care plans (ISP). Manage readiness to start HIV medications, monitor adherence, schedule and re-schedule appointments in EPIC, and ensure information is shared among medical and mental health team members. Provide interpretation when needed, support crisis intervention, participate in grant writing activities related to case management, and engage in weekly clinical supervision and departmental activities. Requires HIV Case Management Certification preferred and at least four years of relevant experience; involves working with diverse communities and bilingual/bicultural capabilities; benefits include comprehensive employee offerings.
Required Qualifications
- HIV Case Management Certification preferred
- Bachelor’s Degree
- Minimum of four years of Human Services delivery experience and/or a Bachelor’s Degree
- Experience working with clients who are HIV positive, have a substance use history and/or have mental health issues
- Certification as a Substance Abuse Counselor preferred
