Job Summary
The Medical Case Management Supervisor oversees staffing and performance of Medical Case Managers and HIV/PrEP Patient Navigators, ensuring compliance with DHH standards and proper training for staff. Key duties include conducting bi-weekly chart reviews, providing leadership and ongoing supervision, collaborating with health network teams for patient needs assessment, and ensuring accurate documentation in EHR systems. Requires experience in medical case management, supervisory skills, and ability to navigate various healthcare regulations.
Required Qualifications
- 2 years of medical case management and/or PrEP care coordination experience required
- Masters’ degree from an accredited university is required or a bachelor's degree in social work, psychology, or sociology, or related field with 3 years of medical case management and/or PrEP care coordination experience
