Job Summary

Conduct outreach to members to gather medication profiles, allergies, and pharmacy information, then document changes and route them to RNs for reconciliation. Schedule and confirm RN joint calls and follow-up appointments while managing reminders and re-attempts per outreach cadence. Assist with Transitions of Care tasks including post-discharge calls, records requests, and follow-up scheduling. Prepare charts for RN assessments and send provider notifications with concise plan highlights. Track referrals and authorizations, request and upload medical records, and verify patient identifiers. Maintain accurate documentation in the EHR/CRM and process Release of Information forms. Coordinate non-clinical services such as transportation and housing resources per care plan guidance. Confirm member eligibility for services and escalate clinical concerns promptly to RNs. Adhere to HIPAA and scope of practice, performing no independent clinical judgment.

Required Qualifications

  • High school diploma or equivalent
  • Valid California driver's license
  • Valid vehicle insurance
  • Must be able to travel using personal vehicle
  • Able to lift up to 10lbs
  • Proficient with EHR/CRM systems and basic payer portals
  • Strong typing and documentation skills
  • Demonstrates strong attention to detail
  • Manages time effectively
  • Communicate with members professionally and compassionately
  • Collaborates effectively with the clinical team and other departments
  • Applies problem-solving skills and resourcefulness
  • Maintains integrity, confidentiality, and strict compliance with policies and standards
  • Must be able to lift up to 10lbs

Desired Qualifications

  • Medical Assistant training/certification (CMA, RMA, CCMA or equivalent)
  • 1–2+ years in ambulatory care, care coordination, or payer/provider support role
  • Medicaid/Medicare populations experience
  • Bilingual Spanish/English
  • Excellent communication (phone, text, email) and customer-service mindset
