Job Summary
Room patients, obtain vital signs, and document medical history while assisting providers with in-clinic visits and health assessments. Support virtual encounters from the clinic, accurately document clinical data in the EHR, and perform medication reconciliation to identify discrepancies. Conduct telephonic outreach for care coordination, manage in-basket messages and prior authorizations, and assist with scheduling, records, and DME coordination. Prepare clinical equipment, collaborate with interdisciplinary team members, and perform clinical tasks within scope including injections and phlebotomy. This role contributes to Upperline Health's value-based care model by supporting preventive care and chronic disease management to improve patient outcomes. Required Qualifications
- Certified Medical Assistant
- minimum of two years' experience in primary care
- Strong knowledge of medical terminology
- basic pharmacology
- Experience with team-based care models
- Care coordination or case management experience
- Experience with adult and geriatric patients
- EMR experience
- AthenaHealth
- Proficiency in Microsoft Office tools
- Reliable transportation
- valid driver's license
- Strong customer service orientation

- Ability to multitask in a fast-paced environment
