Job Summary

Facilitate telemedicine and in-person provider visits for residents in skilled nursing facilities while coordinating care between residents, providers, facility staff, and family members. Prepare residents for encounters, maintain accurate electronic health record documentation, and support transitions of care, referrals, and follow-up appointments. Reinforce provider instructions and approved patient education materials while ensuring compliance with quality initiatives. Maintain reliable on-site attendance and professionalism while representing Theoria Medical within partner facilities across Lima, OH. Local travel up to 30 miles may be required.

Required Qualifications

  • Medical Assistants must have completed an accredited Medical Assistant program
  • Certified Medical Assistant (CMA) certification is preferred
  • Previous experience as a Medical Assistant in a clinical, post-acute care, care coordination, managed care, or geriatric healthcare setting is preferred
  • Superior interpersonal skills
  • Experience charting in an EMR
  • Strong attention to detail
  • Problem solving, thinking autonomously, and owning the solution
  • Professional demeanor
  • Knowledge of geriatric medical practice and terminology
  • Innovative mindset
  • History of successful outcomes or quality-driven practices
  • Commitment to ethical patient care
  • Teamwork and a can-do attitude
  • Advanced computer skills (e.g., Excel filtering and advanced features, Google/Gmail, etc.)
  • Strong communication skills (verbal and written)
  • Local travel may be required, up to 30 miles one way
  • Ability to lift up to 20 lbs. independently and assist with resident transfers involving greater weights as part of a team
  • Ability to stand for extended periods
  • Ability to travel to patient locations (e.g., home, hospital, SNF)
  • Fine motor skills and visual acuity required

Desired Qualifications

  • Certified Medical Assistant (CMA) certification is preferred
  • Previous experience as a Medical Assistant in a clinical, post-acute care, care coordination, managed care, or geriatric healthcare setting is preferred
