Job Summary
Collaborate with departments to ensure employees receive appropriate care and programs, while formulating individualized health interventions based on risk assessments. Assist with annual physicals, screenings, and testing, then coordinate client referrals and follow-up evaluations with detailed documentation. Provide one-on-one counseling, conduct training sessions, and interpret screening results to guide client actions. Ensure a safe working environment by following regulatory policies and maintain effective relationships with staff, providers, and the public. Work within electronic medical records to register patients and document visits, while communicating care barriers to the Pod Manager. May require travel between clinics.
Required Qualifications
- Ability to communicate effectively at all levels throughout an organization
- Ensure business is conducted in a timely, confidential, courteous, and professional manner
- Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, stress reduction and chronic condition
- Optimistic, understanding, innovative, conscientious
- Ability to establish and maintain effective working relationships with colleagues, employees, and the public
- Smart Phone Required - 250 MB for time keeping app
- Medical Assistant (Licensed MA preferred)
- Basic Cardiac Life Support for Healthcare Providers (BLS) - required prior to onboarding (American Heart Association or Red Cross ONLY)
