Job Summary
Capture relevant patient health information while rooming patients, taking vitals, administering point-of-care testing, and performing age-appropriate screening assessments. Assist with post-visit care coordination by scheduling specialist appointments, managing referrals, and sharing information across internal and external care teams. Collaborate with providers to monitor patient panels for preventive measure compliance and participate in daily practice operations including phone triage, email responses, and clinical space upkeep. Coach patients on chronic disease management using motivational interviewing techniques and provide anticipatory preventive guidance to families. This role supports the Senior Health member population (age 65+) at our Mesa, AZ office, focusing on building trusting, empathetic relationships within a primary care team.
Required Qualifications
- Graduated from an accredited postsecondary program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES) or completed relevant Medical Assistant training by any branch of the armed forces
- OR possess a Medical Assistant certification issued by a certifying organization accredited by either the National Commission for Certifying Agencies or the American National Standards Institute
- Strong written and verbal communication skills
- Experience working on collaborative, diverse and feedback-driven multi-disciplinary teams
- A proven track record of persisting through change, demonstrating a forward thinking perspective when under pressure, and consistently stepping up to take action on challenges
- Proficiency in computer technology such as typing, navigating the internet and using multiple software systems simultaneously
- Must be authorized to work in the U.S.
