Job Summary
Health Navigator in a primary care team focusing on seniors (65+): build trusting relationships, perform rooming, vitals, point-of-care testing, and screening assessments; assist with referrals and care coordination; collaborate with providers to monitor preventive measures; support daily operations and front desk inquiries; educate patients on chronic disease management; coach families using motivational interviewing; contribute to a patient-centered in-office experience.
Required Qualifications
- At least 6+ months of experience in a high touch customer service or patient facing role in a healthcare setting
- Graduated from an accredited postsecondary program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES) or completed relevant Medical Assistant training by any branch of the armed forces
- Medical Assistant certification issued by a certifying organization accredited by either the National Commission for Certifying Agencies or the American National Standards Institute.
