Job Summary
Conduct directed treatment for complex injuries and illnesses by driving to patients' homes for guided assessments and supervised care under Advanced Practice Provider direction. Navigate health management conversations, medication goals, and care plans while maintaining accurate medical records in the EMR system. Act as a conduit for Health Plan Partnerships patients, adhering to clinical safety standards and performing phlebotomy. Maintain cultural sensitivity and compassion throughout patient-centered practice, completing required continuing education and training.
Required Qualifications
- Must have one of the following certifications: MA, CMA, CCMA, or RMA
- Valid CA Driver's License (for at least 2 years)
- acceptable driving record
- minimum 2 years of recent in-person experience as a MA, CMA, CCMA, or RMA in an Urgent Care, Primary Care, or related setting
- Phlebotomy experience
- Active and current American Heart Association BLS
- Ability to lift & carry 30-40lbs in a backpack style equipment bag
- Fluent bilingual English/Spanish
Desired Qualifications
- Clinical work experience with pediatrics, preferred
