Job Summary
Collaborate with departments to ensure employees receive appropriate care and programs, formulating individualized health interventions based on risk assessments. Assist with annual physicals, screenings, and testing while documenting results and coordinating client referrals to other providers. Provide one-on-one counseling, conduct training sessions for regulatory requirements, and interpret screening services to guide clients on actions. Consult with internal and external partners for care navigation, build teams and partnerships, and ensure a safe working environment by following organizational policies. Register patients in electronic medical records and document visits thoroughly. Weekdays only, 7 am to 4 pm. Certified Medical Assistant preferred with at least two years of experience.

- Ability to communicate effectively at all levels throughout an organization
- Ensure business is conducted in a timely, confidential, courteous, and professional manner
- Knowledge of specific disease and lifestyle related topics such as smoking cessation, weight management, nutrition, stress reduction and chronic condition
- Optimistic, understanding, innovative, conscientious
- Ability to establish and maintain effective working relationships with colleagues, employees, and the public
- Smart Phone Required - 250 MB for time keeping app
- Basic Cardiac Life Support for Healthcare Providers (BLS) - required prior to onboarding (American Heart Association or Red Cross ONLY)

Desired Qualifications
