Medical Assistant

Medical Assistant role supporting the clinical care team in the field with assigned providers; travel up to 60 miles from home; typical schedule Monday–Friday dayshift with hours 8-hour days; mileage reimbursement offered. Responsibilities include enabling provider focus on patient care, ensuring patients meet medical necessity per protocol, serving as a buffer between providers and families/staff, documenting encounters for CCM and BHI services, updating scheduling rosters, managing day-of changes, preparing clinical documentation, performing medication reconciliation, entering lab and diagnostic results, collecting and sorting clinical paperwork, hosting telehealth sessions, and assisting with discharge planning and transitions of care. Compliance with HIPAA and confidentiality is required. Maintain communications with patients and families, and adhere to organizational policies.
