Medical Assistant - Disease Management Case Worker

Job Summary
Conduct quality assurance chart reviews and perform accurate daily data entry to update and validate the Disease Registry system. Generate disease management reports, including patient visit summaries, and monitor the registry for anomalies to report problems immediately. Provide patient education counseling on nutrition, medication compliance, and lifestyle habits while coordinating resources to help patients achieve optimal disease management. Contact and follow up with patients to ensure program objectives are met, collaborate with clinic staff to improve health outcomes, and provide coverage for other case workers as needed. This role requires flexible scheduling including evenings and weekends, 10% travel outside the local area, and bilingual English/Spanish skills.
Required Qualifications
- Certified Medical Assistant
- Current CPR and/or BLS Certification
- Valid State Identification
- Ability to read, speak and write in English and Spanish
- Reliable transportation
- Must be able to travel from facility to facility
- This position requires 10 % travel outside the local area
- Ability to push, pull and lift up to 20 pounds
- Ability to occasionally push, pull and lift up to 40 pounds
- Stamina to maintain attention to detail despite interruptions
- Ability to communicate in person and over the telephone
Desired Qualifications
- High School Diploma and/or GED equivalent preferred, but not required
- At least one year of full-time paid clinical practical experience (preferred)
- Commitment to goals and philosophy of Northeast Community Clinic
- Bilingual English/Spanish preferred
