Medical Assistant - Disease Management Case Worker

Job Summary
Conduct quality assurance on medical charts, perform daily data entry, and validate information in the disease registry system. Generate disease management reports, coordinate resources for patient care, and provide patient education counseling on nutrition, medication compliance, and lifestyle habits. Contact and follow up with patients to ensure program objectives are met, monitor the registry for anomalies, and report problems to the supervisor. Provide coverage for other case workers as needed and collaborate with clinic staff to improve health management. This role requires flexible scheduling including evenings and weekends, 10% travel outside the local area, and bilingual English/Spanish skills.
Required Qualifications

- Ability to read, speak and write in English and Spanish
- Reliable transportation
- Must be able to travel from facility to facility
- This position requires 10 % travel outside the local area
- Certified Medical Assistant
- Current CPR and/or BLS Certification
- Valid State Identification
- Commitment to goals and philosophy of Northeast Community Clinic
- Ability to work as a team player and work independently
- At least one year of full-time paid clinical practical experience
- Stamina to maintain attention to detail despite interruptions
- Ability to communicate in person and over the telephone
- Ability to use a computer
- Ability to read printed materials
- Ability to move
- Ability to remain stationary
- Ability to regularly push, pull and lift up to 20 pounds
- Ability to occasionally push, pull and lift up to 40 pounds
- Mobility to work in a standard office/clinic setting
Desired Qualifications
- Bilingual English/Spanish preferred
- High School Diploma and/or GED equivalent preferred, but not required
- At least one year of full-time paid clinical practical experience (preferred)
