Job Summary
Coordinate care between specialists, therapy, home health, and ancillary services while managing front desk duties like appointment scheduling. Act as a community liaison between nursing, sales, wellness, and leadership to address facility needs. Greet patients and staff, take vital signs, room patients, and assist with preparation for visits. Complete all diagnostic testing and coordinate after-visit coordination. Manage quality assurance monitoring and conduct monthly outreach with residents and families to assess needs and connect patients with social support services. This role supports Curana Health's mission to improve health outcomes for older adults across 1,500+ communities.
Required Qualifications
- Certified Medical Assistant
- Experience working as a medical assistant in a healthcare setting
- Proficient in Microsoft Office
