Job Summary
Greet patients and staff, take vital signs, and prepare individuals for clinic visits. Coordinate care among specialists, therapy, and home health services while managing front desk duties like appointment scheduling. Perform all diagnostic testing and complete after-visit coordination. Act as a community liaison between nursing, sales, wellness, and leadership to address facility needs. Conduct monthly outreach with residents and families to assess needs and connect them with social support services. Maintain quality assurance monitoring and ensure care aligns with provider plans.
Required Qualifications
- Certified Medical Assistant
- Experience working as a medical assistant in a healthcare setting, preferably in a geriatric or senior care environment
- Proficient in Microsoft Office
