Job Summary

Develop an efficient process for Chronic Care Management by identifying eligible patients, scheduling calls, and following up through collaboration with physicians and office staff. Support identification of eligible patients and complete monthly calls according to best practice guidelines, ensuring accurate coding for maximum financial capture. Provide age-based, culturally, and language-appropriate patient education while acting as a patient advocate to assist patients through the healthcare system. Maintain comprehensive patient files, records, and other information in compliance with HIPAA regulations, and conduct ongoing responses to care plans based on changes in patient status. Notify RN/LVN on new information and ensure all care team members are informed of updates. Comply with system policies and procedures while participating in required meetings and professional development activities.

Required Qualifications

  • High school diploma/GED equivalent
  • Graduation from an accredited program for Medical Assistants
  • Knowledge of community health concepts, principals of community health care, care coordination and epidemiology
  • Knowledge of organizational policies, regulatory requirements and procedures to administer patient care
  • Knowledge of common safety hazards and precautions to establish a safe work environment and evaluate same for home care
  • Skill in health interviewing techniques, teaching and communication both verbally and in writing
  • Skill in identifying problems and recommending solutions utilizing established protocols and a common-sense approach
  • Skill in preparing and maintaining records, writing reports and responding to correspondence
  • Skill in developing and maintaining site quality assurance
  • Skill in establishing and maintaining effective working relationships with patients, medical staff and the care team members
  • Ability to react calmly and effectively in emergency situations

Desired Qualifications

  • Certification from the American Association of Medical Assistants preferred
  • Minimum of 2 years experience preferred but not required
  • Epic EMR experience preferred
  • Bilingual in English/Spanish preferred but not required
  • Candidates should have exceptional people and communication skills

  • Candidates should have a commitment to achieving a high level of accuracy and attention to detail
