Job Summary
Conduct proactive, person-centered support for patients living with chronic medical conditions using population health strategies and risk-based outreach. Identify patients who may benefit from additional support to ensure they receive timely, coordinated, whole-person care responsive to their individual goals, preferences, medical complexity, and social needs. Collaborate within an interprofessional care team to deliver comprehensive assistance.
Required Qualifications
- Graduation from accredited Medical Assistant program
- WA MAC license
- BLS
- Current driver's license and auto insurance
- Immunizations (COVID, Hep B (3), Tdap, MMR (2), Varicella (2), TB/PPD, FLU)
- Pre-employment drug screen
Desired Qualifications
- 3 years related outpatient clinical experience
