Job Summary
Conduct chart audits in Epic to identify open care gaps across quality measures, verify documentation support, and track closure status. Assist with root cause analysis of quality gaps by gathering data, participating in discussions, and documenting corrective action follow-through. Perform patient outreach via phone, MyChart, or mail to close care gaps, educate on screenings, and address barriers to care. Manage episodic care needs by identifying recent Emergency Department visits, conducting post-ED outreach to assess status and discharge understanding, and coordinating follow-up appointments. Escalate urgent clinical concerns to supervising providers and document all activities accurately in Epic. This role works closely with Care Managers, providers, and the Practice Transformation team to ensure timely, well-coordinated care across assigned clinic panels.
Required Qualifications
- Active Registered Medical Assistant (RMA) or Certified Medical Assistant (CMA) certification in good standing or the ability to obtain within 6 months of employment
- Prior experience in a clinical, medical assistant, or care coordination role
- Working knowledge of electronic health record systems (Epic preferred)
- Strong verbal and written communication skills, with the ability to engage patients empathetically by phone and in writing
- Strong organizational skills with the ability to manage multiple concurrent audits, outreach lists, and program requirements
- Comfort working within defined RMA/CMA scope of practice under National and State regulations and clinic policy
Desired Qualifications
- Experience with quality measure programs such as HEDIS, ACO, or Collaborative
- Familiarity with chart audit processes and root cause analysis methodology
- Experience supporting quality improvement or practice transformation initiatives
