Job Summary
Coordinate, transport, and attend all medical care appointments for residents in assigned group homes while administering medications and updating logs accurately. Provide direct care and support for adults with developmental disabilities, including assisting with personal hygiene, domestic duties, and developing basic living skills through instruction. Maintain a safe environment by adhering to behavior and health management plans, reporting safety concerns, and documenting all changes in resident status. Communicate and advocate with medical providers, nurses, and program staff regarding treatment plans while managing resident and agency funds per policy. Serve as a role model and maintain positive relationships with residents, families, and service providers to support independent living goals.
Required Qualifications
- Valid NY/NJ/CT/PA Driver's License with 6 points or less
- High School / G.E.D.
- Must maintain current certification as required by The New York Foundling and the residents Individual Service Plan (e.g., CPR, First Aid, AMAP, SCIP-R)
