MEDICAL ADHERENCE NURSE CARE MANAGER

Job Summary
Provide primary and specialty care to patients with chronic illnesses, focusing on adherence issues, self-management, and harm reduction. Develop care plans, refer patients to internal and external resources, and monitor medical progress while maintaining data integrity through accurate, confidential record-keeping. Conduct sensitive interviews, prepare comprehensive reports, and participate in quality improvement activities within a multidisciplinary team. Requires active RN licensure in DC, knowledge of HIV/AIDS and substance abuse, and flexibility to work various shifts across NW and SE DC locations.
Required Qualifications
- Bachelor of Science in Nursing or Equivalent work experience
- Active licensure as a Registered Nurse in the District of Columbia
- Ability to work harmoniously with diverse groups of individuals
- Flexibility to work various shifts, including afternoon, evening, and weekend shifts as necessary
- Flexibility to work within the organization's various locations as necessary
- Commitment to health equity
- Fluency in medical terminology
- Knowledge of healthcare information systems with the ability to adapt to new electronic medical records and population health platforms
- Analytical skills necessary for the preparation of accurate, concise, and comprehensive reports
- Ability to maintain records and information in an accurate, timely, and confidential manner
- Ability to adapt to changes in the service delivery model, re-prioritizing as necessary
- Ability to work independently with minimal supervision
- Ability to manage time and effort in order to meet strategic objectives
- Knowledge of principles and processes for providing excellent customer service, both internal and external
- Knowledge of all applicable WWH policies and procedures
- Excellent oral and written communication skills
- Ability to assess patients for needs related to treatment education, risk reduction, or prevention
- Highly developed communication skills, including the ability to function efficiently as part of a multidisciplinary team in providing patient care
- Ability to conduct sensitive, empathetic interviews that respect the dignity and diversity of patients
- Ability to prepare accurate, concise, and comprehensive reports
- Ability to maintain patient records and information in an accurate, timely, and confidential manner
- Knowledge of HIV/AIDS-related illnesses and substance abuse, including current treatment and adherence strategies
- Knowledge of risk reduction intervention strategies, including harm reduction and motivational interviewing principles
- Knowledge of chronic health conditions, such as diabetes, hypertension, heart disease, etc., with an understanding of the needs of individuals and families across the primary care continuum
Desired Qualifications
- Master of Science in Nursing or Public Health
- 1-3 years of experience post RN licensure
- Experience working with an ethnically, culturally, and racially diverse work staff
- 1 year of experience working with people living with HIV or issues related to HIV care
- Knowledge of eCW EMR
