Job Summary
Coordinate specialized support for families facing complex problematics by analyzing situations, determining necessary assistance, and executing crisis interventions. Develop detailed plans of action, establish safety plans, and monitor the safety, progress, and effectiveness of ongoing support. Collaborate closely with the social team, chain partners, and other professionals to ensure cohesive care and manage agreed-upon appointments. Regularly evaluate interventions and adjust support as needed to guarantee families receive appropriate, effective assistance. Maintain comprehensive oversight of case management and family well-being throughout the trajectory.
Required Qualifications
- Een afgerond hbo-diploma in Social Work, SPH, MWD, Pedagogiek of een vergelijkbare richting
- Een geldige SKJ-registratie
- Ervaring met complexe gezinssituaties en crisisinterventies
- Kennis van veiligheidsvraagstukken en Veilig Thuis-procedures
- Ervaring met het opstellen van veiligheidsplannen en plannen van aanpak
- Sterk in analyseren, plannen en het vertalen van inzichten naar concrete acties
- Kunt regie voeren en verschillende betrokken partijen met elkaar verbinden
- Communicatief vaardig en werkt gemakkelijk samen met het sociaal team en ketenpartners
- Proactief, oplossingsgericht en weet ook in complexe situaties het overzicht te bewaren
- Oog voor veiligheid, zorgcoördinatie en het welzijn van het gezin
