Job Summary
Monitor resident health and well-being by administering medications, tracking vital signs, and documenting clinical care changes through the Personalized Service Plan process. Support the Wellness Nurse in completing monthly wellness visits and inform leadership of medication or service level adjustments requiring tier upgrades or potential moves. Identify ongoing resident needs, assist with daily care tasks, and maintain clinical quality assurance while demonstrating leadership capabilities in medication administration and resident support.
Required Qualifications
- Must be certified Medication Aide and/or meet the state requirements for medication aide
- Must have demonstrated Leadership capabilities
- Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
Desired Qualifications
- A minimum of one year of experience working with elders or disabled individuals, preferred
- Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
