Job Summary
Monitor resident health and well-being by assessing needs through the Personalized Service Plan process and ensuring proper documentation for clinical care changes. Support medication administration and assist with obtaining monthly vital signs and weights prior to wellness visits. Complete clinical sections of monthly wellness visits and PSPs alongside the Wellness Nurse, while communicating medication or service level changes to the Wellness Director or Executive Director. Inform leadership of any potential tier level increases or move-outs. Maintain clinical quality assurance while demonstrating core Phoenix program wellness elements.
Required Qualifications
- Must be certified Medication Aide and/or meet the state requirements for medication aide
- Must have demonstrated Leadership capabilities
- Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
Desired Qualifications
- A minimum of one year of experience working with elders or disabled individuals, preferred
- Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
