MDS Specialist

Assists the Director of MDS in the development, completion and transmission of the resident assessment in accordance with the state requirements and policies and goals of the facility. Participates in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care. Ensures that care plan includes measurable objectives and timetables to meet resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in assessment, and that schedule of activities is developed as required. Demonstrates commitment to the organization's mission and values, promotes teamwork, respects residents' rights, and complies with all policies and procedures. Provides assistance to co-workers and ensures residents receive fair, dignified, and respectful care. Knowledge of long-term care regulations and Medicare participation; ability to contribute to high-quality, person-centered care with integrity and compassion.
