Job Summary

Coordinate the Resident Assessment Instrument and MDS processes, including developing interdisciplinary schedules for assessments and care plan reviews. Ensure assessments are completed accurately within timeframes by reviewing medical records for documentation support and coding accuracy. Collaborate with nursing, therapy, dietary, and social services to obtain assessment information and identify significant resident condition changes requiring updates. Monitor Medicare eligibility, skilled coverage, and certification requirements while tracking submissions, corrections, and completion deadlines. Conduct regular audits of MDS coding and care plans, reporting trends and findings to nursing leadership and the Quality Assurance committee. Provide education on MDS, RAI, and reimbursement requirements to clinical personnel while maintaining resident privacy and compliance with federal and state regulations.

Required Qualifications

  • Graduate of an approved practical or registered nursing program
  • Current, active, and unencumbered RN or LPN license in the State of Missouri
  • Current BLS/CPR certification
  • At least 18 months of recent experience as an MDS Coordinator
  • Experience with clinical reimbursement and Medicare documentation
  • Working knowledge of the MDS, RAI process, care planning, and skilled nursing documentation
  • Skilled nursing or long-term care experience
  • Strong clinical assessment, critical-thinking, communication, and organizational skills
  • Strong attention to detail and commitment to accurate documentation
  • Ability to manage multiple assessments, deadlines, and competing priorities
  • Ability to communicate expectations and provide education to interdisciplinary team members
  • Proficiency with electronic health records and MDS-related software
  • Dependable attendance and reliable transportation
  • Professional and collaborative demeanor
  • Commitment to treating every resident with compassion, dignity, and respect
