MDS (RN/LPN) Coordinator

Coordinate MDS and PPS assessments, verify Medicare eligibility, ensure continuing stay documentation, oversee the RAI process and care plan updates, collaborate with the Interdisciplinary Team, monitor MDS coding and documentation, assist with identifying significant changes and physician orders to reflect updates, support trend reporting to the Quality Assurance Committee, and maintain confidentiality of necessary information.
