Job Summary
Coordinate MDS, CAAS and care planning for residents, ensuring timely assessments and care plans aligned with state/federal regulations; conduct assessments, document accurately, and communicate with the interdisciplinary team; participate in Quality Improvement/Assurance activities and regulatory surveys; mentor MDS nurses, support documentation training for MDS and EHR systems, and assist in forecasting revenue; provide resident-centered care, advocate for residents and families, and maintain professional standards in documentation and care planning.
Required Qualifications
- Current unrestricted license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in practicing state
- 2+ years of MDS experience
- A nursing degree from an accredited college or university
