MDS Registered Nurse (RN) - PRN

MDS Nurse coordinates MDS, CAAS and care planning for residents, conducts assessments, documents and communicates ARDs with the interdisciplinary team; maintains care plans within regulatory guidelines; supports quality improvement, training on MDS/EHR documentation, and mentors MDS nurses. Position is PRN (as needed) in a round-the-clock skilled-nursing environment in Lindenhurst, IL; requires strong communication and proactive resident-care coordination.
