MDS Registered Nurse (RN) - PRN

MDS Nurse (RN) PRN responsible for coordinating and completing resident assessments and care plans (MDS/CAAS) per state/federal guidelines, including ARD scheduling, interdisciplinary collaboration, and ensuring accurate nursing documentation. Key duties include managing MDS coordination at admission, maintaining ARD timelines, documenting assessments, supporting quality improvement/assurance programs, preparing for inspections, and mentoring other MDS staff. Requires a nursing degree and an active RN or LPN license with 2+ years of MDS experience; strong communication, assessment, and organizational skills; travel not required. Located in Lindenhurst, IL or Ingleside, TX with in-person responsibilities.
