Job Summary
MDS Nurse (RN/LPN) PRN position at Village at Victory Lakes in Lindenhurst, IL. The role centers on organizing, completing, and evaluating resident assessments and care plans (MDS, CAAS) per state/federal guidelines and coordinating with the Director of Nursing. Key duties include coordinating the MDS and care planning process, monthly Medicare/Managed care reviews, documenting assessments, establishing ARDs, collaborating with the interdisciplinary team, participating in quality improvement programs, surveys, and maintaining compliant nursing documentation. Requires a nursing degree, an active LPN or RN license, and 2+ years of MDS or skilled nursing experience. Strong communication skills and the ability to mentor MDS nurses are desired; travel is not indicated. Hourly pay range is $39-$42. Location: North Lindenhurst, NY (US).
Required Qualifications

- Licenses/Certifications: Current unrestricted license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in practicing state
