MDS Nurse (RN or LVN) FT

Full Time MDS Nurse Coordinator position responsible for conducting comprehensive resident assessments, documenting medical histories, and developing individualized care plans in collaboration with interdisciplinary teams. Participate in care conferences, ensure regulatory compliance, and provide guidance on best practices in resident care. responsible for monitoring resident conditions and facilitating accurate coding for reimbursement. Engage with residents’ families with empathy and professionalism, fostering a customer-centric care environment. Requires an RN license and at least two years of MDS experience in long-term care, along with strong communication, documentation, and computer skills. This on-site role is based in Mesquite, TX and emphasizes collaboration, quality care, and professional growth.
