Job Summary
Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA, and other payer program requirements. Ensure assessments accurately reflect the physical, mental, and psychosocial status of each resident while maintaining appropriate documentation to support services provided. Participate in the pre-admission process to obtain essential information for MDS/Case Mix optimization and collaborate with the interdisciplinary team to facilitate efficient care plan development. Track skilled customers utilizing Case Management Tools to determine continued Medicare/Managed Care eligibility and benefit periods, while gathering information for Utilization Reviews and communicating with external Case Managers. Conduct regular audits of the MDS process, including validation of coding documentation and evaluation of Data Integrity Audit reports. Maintain an accurate schedule of all MDS assessments, perform concurrent reviews to assure appropriate RUGs categories, and ensure timely electronic submission of Minimum Data Sets. Assist in the orientation and training of new associates on the RAI process and disseminate updated materials regarding Federal and State regulations.
Required Qualifications
- Must be a graduate of an accredited school of nursing
- Current R.N. or L.V.N.
- Licensure in the state in which employment occurs
- Minimum of two years health care experience
- Must possess superior clinical assessment and documentation skills
- Must demonstrate strong interpersonal skills and ability to work well in a team environment
- Ability to work flexible work hours to support business requirements
- Ability to utilize both local and corporate resources in the execution of job responsibilities
- Must possess excellent verbal and written communication and presentation skills
- Must possess the ability to efficiently and effectively lead interdisciplinary teams and coordinate and manage RAI process
- Must possess the ability to effectively communicate
