Job Summary
MDS Nurse overseeing resident assessments and care planning in a Pennsylvania skilled-nursing facility. Responsible for conducting and coordinating resident assessments, developing and implementing care plans with the interdisciplinary team, ensuring timely completion of assessments within fourteen and twenty-one days of admission or change in condition, and aligning care with residents' needs and facility policies. Requires graduation from an accredited nursing program and PA RN/LPN licensure; prior MDS experience and background in skilled nursing or acute care are highly preferred. Supervisory potential noted as an opportunity for RN/LPN to advance.
Required Qualifications
- Graduate of an accredited Nursing program required
- Licensure and registration by the PA Board of Nursing Examiners as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required
- Prior experience in a skilled nursing care facility, acute care setting, or other related field preferred
- Previous MDS experience highly preferred
